Guidelines for the Reference Rating Form and Letter of Support
 

A person who provides references for the applicant (i.e., a reference) must complete the Reference Rating Form and write a letter of support.  The list of individuals who serve as references should include a major academic advisor (e.g., a thesis advisor), a long-term instructor (e.g., someone from whom the student has taken multiple classes), and a supervisor (e.g., a clinical supervisor or job supervisor). 

The Letter of Support and the Reference Rating Form completed by each individual serving as a reference are confidential and should not be shared with the applicant.  References must make a copy of the reference rating form and letter of support and must place the original and the copy each in its own envelope.  Each envelope must be sealed and the reference must sign across the closure of each envelope to assure confidentiality and prevent tampering with the documents.  The envelopes must be returned by the reference to the applicant for inclusion in the application packets. 

NOTE:  One copy of each reference letter and signed, sealed Reference Rating Form must be included in the application packet that is submitted to the UAF Ph.D. Program Coordinator and one signed, sealed copy of each reference letter and Reference Rating Form must be included in the application packet that is submitted to UAA Ph.D. Program Coordinator.  It does not matter which Office receives the original versus the copy.
