Guidelines for the Reference Rating Form and Letter of SupportA person who provides references for the applicant (i.e., a reference) must complete the Reference Rating Form and write a letter of support. The list of individuals who serve as references should include a major academic advisor (e.g., a thesis advisor), a long-term instructor (e.g., someone from whom the student has taken multiple classes), and a supervisor (e.g., a clinical supervisor or job supervisor). The Letter of Support and the Reference Rating Form completed by each individual serving as a reference are confidential and should not be shared with the applicant. The rating form and letter must be sealed and the reference must sign across the closure of the envelope to assure confidentiality and prevent tampering with the documents. The three reference envelopes may be returned by the reference to the applicant for inclusion in the application packets or they may be mailed directly to the UAA Office of Admissions.
